Hospital Admission Toolkit for People with Disability

Being admitted to hospital can be stressful, but even more so for people with disability, their family and carers.

In order for hospital staff to look after the patient, they need to ask them a lot of questions, and this is when things can get complicated. Often, this experience can be worrying, confusing and frustrating for people with disability who may have difficulty in communicating. This can then lead to misdiagnosis, extended stay in the hospital and other issues. 

This is when the Admission2Discharge (A2D) Together Folder for People with a Disability can come in handy. The folder is a useful toolkit for holding crucial information related to the person's health.

Information that can be held in the A2D Together folder include the person's:

  • Full Name and preferred name;
  • Guardian or carer's name and contact details;
  • Medication files and reminders;
  • Important notes (e.g their ways for communicating, things they like or do not like, things they need assistance with); and 
  • Support, management, and hospital plans.

The A2D Together Folder is an initiative of the Department of Family and Community Services (FACS), Supported Accommodation Services Team, South East Sydney District, the Metro-Regional-Intellectual-Disability Network (MRID) and Carer Program, South Eastern Sydney Local Health District (SESLHD).
