New resources for people with disability staying in hospital

What you need to know

  • The Ready to Go Home project has launched the My Hospital Pack and a series of Your Hospital guides to assist people with disability to have a well-prepared and positive hospital stay.
  • The seven short resources have been codesigned by people with lived experience of disability and multiple hospital stays.
  • The packs and guides are free and printed versions can be ordered up to Friday 17 April. Or you can download them from the NDS website.

The Ready to Go Home project has launched My Hospital Pack and a series of Your Hospital guides to assist people with disability to have a well-prepared and positive hospital stay.   

The Your Hospital guides are seven short resources co-designed by people with lived experience of disability and multiple hospital stays. The My Hospital Pack has been co-developed with Joondalup Health Campus, north-metro service providers, and consumers. 

The My Hospital Pack is an easy-to-use envelope with two checklists. Ready for Hospital is to be completed in the community (generally, at home) and Ready for Home is to be completed by hospital staff as a discharge tool. 

The Your Hospital guides, which are available in A5 booklet, easy print, Easy Read and accessible Word versions cover: 

  1. Be prepared for a hospital stay – plan, prepare, pack and prioritise 
  2. Your hospital stay – what might happen, who you might talk to and how you might feel 
  3. Decision-making during a hospital stay – be prepared, be confident and be comfortable 
  4. Discharge planning checklist – your health, transport, support needs, medications, equipment and home modifications, appointment and final checks 
  5. Your discharge summary – read, review and request 
  6. After your hospital stay – taking extra care, finding additional support and seeking advice 
  7. NDIS basics – starter information for people new to the NDIS and may need to consider applying for the NDIS while in hospital.  

Individuals and organisations can order free My Hospital Packs with Your Hospital guides. Orders will close on 17 April 2023.

When the orders are posted you’ll get a mail-out notification from the RTGH project team. If they wish, organisations may place multiple orders for delivery to multiple addresses (for example, to coordinators at group homes). Alternatively, you can download all the resources from the Ready to Go Home page on the NDS website.
